In providing consent, the parent will be asked to provide his or her first and last name and email address. The parent may choose to grant consent by responding to the email. If the parent does not consent, or if the parent does not respond to the notice within ten (10) days from the date the notice was sent, HealthTeacher will delete the user’s contact information from its records.
